To add a feedback form to a page
You can add forms to your web pages that visitors can fill out, and the results will be mailed to you.
1. Click in the page where you want the form.
2. On the menu bar, choose Insert > Form. The CuteSITE Builder Form Wizard appears.
3. Follow the instructions in the Wizard and click Finish. A Form Plug-in placeholder will appear on the
page. You will be able to see and use the actual form when you preview or publish your site.
To edit an existing form
1. Double-click the Form Plug-in placeholder. The CuteSITE Builder Form Wizard appears.
2. Click Back or Next to find the page in the Wizard where you want to make changes.
3. Make your changes.
4. Click Finish. Your changes will be visible when you preview or publish your site.

To add a form that sends database-ready information
You can set your forms to send reply information in comma separated format, to make it easier to import the
information into a database.
1. Click in the page where you want the form, or open an existing form (see previous page).
2. On the menu bar, choose Insert > Form. The CuteSITE Builder Form Wizard appears.
3. Choose your form type and click Next. Step Two: Enter your e-mail address appears.
4. Enter your email address.
5. Click Form Action Options. The Form Action Options window appears.
6. Choose Comma-separated (for database import).
7. Click OK. The Form Action Options window disappears.
8. Click Next and complete the rest of the Wizard.

To add custom fields to a form
1. Click in the page where you want the form, or open an existing form (see page 91).
2. On the menu bar, choose Insert > Form. The CuteSITE Builder Form Wizard appears.
3. Follow the instructions in the wizard and stop at Step 4: Customize the Form.
4. Select the [Custom1] check box.
5. In Field Name (not visible to users), type any name you want. This name will show up in the email
messages you receive when the form is completed and submitted by a visitor.
6. In Field Prompt (appears on your form), type a name so that visitors will know what information you
want them to enter in this particular box.
7. (Optional) Select the [Custom2] check box, and create another custom field. Every time you select a
[Custom] check box, a new [Custom] check box will appear, until there are a total of twelve custom
fields.
8. Click Finish.
To change the size of a form box
1. Double-click the Form Plug-in placeholder. The CuteSITE Builder Form Wizard appears.
3. Click Next or Back to get to Step 4: Customize the Form.
4. Select the check box for the form box that you want to lengthen.
5. In Field Width, type a whole number. This will be the approximate number of characters that will show in
the box.
6. Click Finish.
To change how many characters a form box will hold
1. Double-click the Form Plug-in placeholder. The CuteSITE Builder Form Wizard appears.
3. Click Next or Back to get to Step 4: Customize the Form.
4. Select the check box for the form box that you want to change.
5. In Max No. of characters type a whole number. This will be the most characters a visitor will be able to
type in the box.
6. Click Finish.

To change the order of boxes in a form
1. Double-click the Form Plug-in placeholder. The CuteSITE Builder Form Wizard appears.
3. Click Next or Back to get to Step 4: Customize the Form.
4. Select the check box for a form box that you want to move.
5. Click the up arrow button to move the selected box before the previous box, or click the down arrow to
move the selected box below the following box.
6. Click Finish.
